According to the gvo.gamedb section on the Rome dungeon loot table: Middle level, floor 5, "Items such as (At the time of hierarchy clear)". I haven't unlocked the middle floor as of yet, but the translated wording makes it sound like a boss drop. I'll leave a link (looks a bit odd, but I had no trouble pasting it into a translation engine) as well:

Confirmed,it is Rome dungeon,middle level,floor 5 , boss drop.
