Just NPC's ambushes from ships you cant see, NPC ships that you can see sailing around can still attack you

Caution can level when not moving, maybe slower though. This means you can "grind" caution afk at port (and frugality while you're at it).

Is really important other for the tenochtitlan quest? (require r10 caution).. is there another hard quest like that that require caution out there?

I've heard this helps a little against visible npcs too, though I always thought it was only to counter ambushes. The skill description is kind of vague about it so is there any source that can confirm whether or not it actually does reduce visible npc assaults?

It doesn't. It is only meant for ambush (NPCs that re NOT visible). This ship skill
Military Camouflage is the one that decreases attacks from visible NPCs.

Speaking from experience Military Camouflage works great for decreasing the attacks from visible NPC fleets.

There's any item / consumable that activate this skill? Like Quadrant does with surveying, or salvage ropes does with salvage...
just i would not need to get this skill when grinding row with my adv alt.

No item gives this skill.
But you could put
High Lookout on your row grind ship, does the same.

Technically correct. However, there are a few items which act as a substitute for Caution skill.
1. Equipment with the EX Effect
Reduce incidence of attacks - this skill effect means invisible ships do not ambush you and there is a chance (based on EX skill rank) that visible ships will not attack you either. I believe R5 Quarters ornaments can give a similar effect but I do not use R5 Quarters and cannot say for certain.
I think this equipment is the best to acquire as it is not UWC:

Decorative Small Boat Blueprint is a consumable which can prevent NPC ship attacks for a 24-hour period. It's not easily acquired however.
3. Pets with the
Area caution skill work much the same way, preventing invisible ship ambushes and occasionally preventing visible NPC attacks. Taiwan Magpie and Wombat are freely acquired from quests; Sea Lion and Capybara are UWC only. Note that you must have
Pet Training skill to activate pet skills, however.
There are many options as you can see. For row grinding, a pet seems the best course unless you like to use master smith tools to constantly repair a weapon slot item.
