Why does "production fail" sometime occur when I'm converting mineral into ore? I'm casting R11 so I don't think its a skill issue.

All recipes with a chance of failure (ie 0-2 ore) will still occasionally fail no matter how high your level. Improving odds of great success (Aide in paymaster job with high stat, produce at sea in a ship with Workshop skill, Magician tarot in effect) should at least compensate for this, if not reduce failures.
