Found this in a treasure chest on Floor 14. It may come from other chests across the Lower Giza levels, as well.

Grinding lower Giza for a few hours netted four of these - all were on 14th floor. So, for you MA completionists, I would focus on 14th floor for this one and the 15th for the other three from the lower level.
