Country of Steel quest from Seville. And, yes, you can get the sword every time you do the quest.

None. Normally, EQ rewards are given to you upon making the discovery, too...

You get the equipment when you do the discovery, not when reporting. Also, a lot of these items are hard to be found again when repeating quests.

I had same problem with amulet of the sun quests.
